FOR CLIENT’S INFORMATION Known (potential) risks associated with tattooing: • Scarring • Blood poisoning (septicaemia) • Localised infection • Allergic reaction to pigment • Localised swelling around the site • Fainting and seizures ⸻ Individual Consent I declare that I give my full consent for tattooing to be carried out by the practitioner named above. I confirm that I understand potential complications may occur (e.g. infection, swelling), and that aftercare advice will be provided following my session. I accept that it is my responsibility to read and follow this advice carefully until the tattoo has fully healed. If aftercare guidance is not followed and the tattoo does not heal properly due to negligence, I understand that a touch-up session may incur a charge. I confirm that all information I have provided is correct to the best of my knowledge. I confirm that I am over the age of consent for this procedure (i.e. 18 years old) and not under the influence of alcohol or drugs. If I am getting a script tattoo, I understand it is my responsibility to check that all lettering is correct before tattooing begins. I acknowledge that it is not reasonably possible for the artist to determine whether I may have an allergic reaction to the pigment or processes used, and I accept that such a reaction is possible. I understand that variations in colour and design may occur between my chosen design and the final result on my skin. I accept that on darker skin tones, certain colours may appear less bright. To my knowledge, I do not have any physical, mental or medical conditions which might affect my wellbeing as a result of having a tattoo. I consent to Buzz Club providing me with a copy of this form along with an aftercare link, which I agree to read thoroughly.
